Credit applications must be completed where applicable, dated, and signed (no digital signatures) by the owner(s) on both the 1st and 3rd pages.
Alterations to the original form will also not be accepted.
If your company does not have credit privileges at any other companies, please write N/A in the Credit Reference section.
Failure to properly complete the application may cause unnecessary delay in processing your request and can result in failure to acquire an account.
BOZ will email written notification on the status of all new Credit applications, usually within ten business days. C.O.D. accounts are usually set up by the following business day.
The completed forms may be returned either in person, by fax to 905-508-2183.
Emailing credit forms to Receivables is not recommended but the option is yours. Please call for the correct email address.
** Boz Electric does not accept personal applications. Any company applying for a Credit account must be registered.
